OPTION 1
Provide an Insurance Binder from your auto insurance company. Please contact our office and request the Insurance Binder Request Form. Have your insurance agent complete and sign the form, along with the actual binder, and email or fax it to our rental office.

Purchase Trailer Rental Insurance through MBA Choice Insurance by following the steps below:

Verify your Rental Dealer Participation |
|
- Enter our MBA Rental Dealer policy number: 421023.
- Click the Next Step button to continue.

| STEP 2: |
Trailer Series Selection |
|
Trailer rental insurance is based on the value of the rental unit you are renting.
Select the applicable Series Number from the list below and click the Next Step button.
- Select Series A if you are renting a Enclosed Trailer or Utility Trailer - $3.00 per day.

Select Series B if you a renting a Travel Trailer or Toy Hauler - $6.00 per day.

Select Series C if you a renting the 28' Airstream Travel Trailer - $10.00 per day.

- Select the check box below our name verifying that we are your RV rental dealer.
- Select the departure date and return date of your Motor Home rental. (Coverage Period)

Provide Renter Information
- Name, address and contact information.
- Click The Next Step button to continue.

Acceptance of Terms & Payment Information |

- Acknowledge the purchase terms and conditions by selecting the I accept the terms and conditions check box.
- Enter your credit card information.
- Click The Next Step button to continue.

Print Certificate |

- Verify that your transaction was successful.
- Your Choice Certificate will be emailed to yourself as well as our rental office.
- You can also print a copy of your MBA Choice Insurance Certificate.
